most likely the form change was lady blackhearts doing, as we didn’t see her fiddling with the book beforehand (we did however see blackheart help design her costume, perhaps with a few enchanted pieces thrown in? at the least she most likely selected the spell out of the book.)
She was carrying the book while going up the stairs from Nox.
This concurs with the general theory of the book containing a shape-shifting spell amongst other spells.
